Prostate enlargement

Also known as benign prostatic hyperplasia (BPH), it is a non-cancerous increase in the size of the prostate gland, which can obstruct urine flow.

Symptoms
  • Increased frequency of micturition
  • Urgency to void
  • Nocturia (getting up in night to void)
  • sense of incomplete emptying after voiding
  • poor flow of urine
  • intermittent stream of urine
  • straining while passing urine.
  • Hematuria (blood in urine)
  • Retention of urine (complete blockage or inability to pass urine)

  • Ultrasound scan
  • Urine tests
  • Uroflowmetry ( if patient can pass urine )
  • Renal function tests
  • Serum PSA ( to screen for possibility of prostate cancer)
Options
Medical management

Tablets which are to be taken daily as per doctors advise. They help in relaxing the bladder outlet enabling easy outflow of urine. Strict compliance needs to be ensured otherwise there can be worsening or recurrence of symptoms. Periodic follow-up in the form of uroflowmetry and ultrasound is needed.

Surgical management

Need for surgical treatment is decided by urologist based on clinical presentation of the patients. There are various surgical techniques of which the commonest modalities offered by us are discussed below.

  • TURP

    A small instrument called resectoscope is introduced into the urethra. Using an electrically heated thin metal wire small pieces of prostate tissue are cut and removed

  • ThuFLEP (Laser prostatectomy /enucleation)
    A small scope is introduced into the urethra. Using LASER , prostate is separated from the surrounding capsule , cut into pieces and removed.

After both procedures , a catheter is placed into the bladder for a short duration and removed .

Decision about the surgical treatment is tailored on individual patient wise basis after a detailed discussion with urologist regarding the pros and cons , complications , expected duration of recovery , post operative precautions .
